To enhance the user experience while using the Services, we may require you to provide personally identifiable information for reasons including, but not limited to:

  • Purchasing InclusionMD products and/or services;

  • Obtaining material information about InclusionMD products/services;

If you choose not to provide the personal information we reasonably require, it may hinder InclusionMD’s ability to provide the Services or information you request. If you create an account on our site, we store your name, address, email, and phone number, which will be used to populate the checkout for future orders, as well as credit card information if you decide to purchase a renewable subscription-based product or service. We will also store comments or reviews if you choose to leave them.

Log Data

InclusionMD collects information that your browser sends to us that is called Log Data whenever you visit our site. Log Data may include information such as the following:

  • Your computer’s Internet Protocol (“IP”) address;

  • The browser and version installed on the device you are using to access our site;

  • Responding to requests e.g., returns, refunds or complaints;

  • Payment Processing;

  • Fraud Prevention;

  • Account setup for our ecommerce platform;

  • Federal, State and Local compliance with the laws and regulations governing taxes, shipping, sale, and distribution of products and/or services;

  • Enhancing store and product offerings; and/or,

  • Enhance communication between InclusionMD and consumers via email marketing;

InclusionMD retains personal information for as long as the information is needed for the intended purpose—that is, for as long as is reasonably necessary for full legal compliance. We share information with third parties who help us provide our orders and store services to you. We accept payments through PayPal, Stripe, and other processing companies. When processing payments, some of your data will be passed to PayPal or Stripe, including information required to process or support the payment, such as the purchase total and billing information.   • Order information for 10 years for tax and accounting purposes: this includes your name, email address and billing and shipping addresses.

  • Products you have previously viewed: we will use this to, for example, show you products you have recently viewed.

  • Shipping address: we will ask you to enter this so we can, for instance, estimate shipping before you place an order, and send you the order; and,

If you are logged in to your YouTube account, YouTube allows you to associate your browsing behavior directly with your personal profile. You can prevent this by logging out of your YouTube account. Google Web Fonts

For uniform representation of fonts, this website uses web fonts provided by Google. When you open a page, your browser loads the required web fonts into your browser cache to display texts and fonts correctly. For this purpose, your browser must establish a direct connection to Google servers. Thus, Google becomes aware that our web page was accessed via your IP address. Children’s Privacy

Our Services do not address anyone under the age of 15. We do not knowingly collect personal information from anyone under 15 years of age. In the case we discover that an individual under 15 years old has provided us with personal information, we immediately delete this from our servers. If you are a parent or guardian and you are aware that your child has provided us with personal information, please contact us so that we will be able to do necessary actions.

Rights

It is your right to request details about personally identifiable information that InclusionMD retains about you. Further, if there is an error contained within the information that is held about you, or if you believe the information is incomplete or inaccurate, please contact us to have it corrected at info@inclusionMD.com or write us at the address below. We will do everything we can to fix the information promptly.

It is also your right to object completely to the collection, retention, or use of your personal information; to request that we have it removed from our site and servers; or to request a copy of the information retained about you on our site for your own personal records.
